Welcome to the Arizona CERT online Medication Record
This tool will allow you to accurately and easily enter all the medicines and supplements you are using and print or save a record to your own computer so that you can share the information with your physician or pharmacist.
Before we begin, be sure to gather all the medicines you are currently taking. Don't forget to include over the counter medicines like aspirin and antacids, as well as all herbs and dietary supplements you may be taking.
