How to Submit a Case to the QT Registry
Instructions for Submissions:
The following instructions will help guide you through the registration process. For further assistance please contact us by e-mail. If you'd prefer someone to call you regarding your case, please let us know.
| Step 1: | as a physician or other professional |
| Step 2: | will be created for the case you are submitting to the registry. Submit each case for a patient only once. |

| Once your Submission Packet arrives follow steps 4 - 7. | |

NOTE: An AzCERT staff member must obtain patient consent in addition to the signed consent form. A phone call is best, but e-mail may be an acceptable method to obtain consent in some circumstances. The Western Institutional Review Board (WIRB) requires that all who obtain consent must have completed training in Protection of Human Subjects. |
